1. The application form must be filled by the candidate only.
2. The application form must be filled by CAPITAl/BLOCK letter only.
3. Write clearly & legibly with NO OVERWRITING/SCRIBBLING/USE OF WHITE FLUID.
4. Use black/ blue ball pen only.
5. The application form must be accompanied by the attested copies of the requisite qualification certificates.
6. Paste one recent passport size color photograph on the application form.Attach two additional; recent Passport Size color photographs with form, use clips (Do not staple pin)..Write your name & From No. behind additional photos.
7. Father's Name/Husband's Name and other details should be as per matriculation certifacate.
